To start off with; I'm sure you can find the book in a library, it's not a new one but they even made it into a movie so it shouldn't be much of a problem.
Now, I read you're respons to Oni's review, are you trying to make a Lost-scenario here...? Questions, questions, questions, and never the answers you want at the right time? Haha, this far you're making a wonderful job! Who is the voice calling him a killer?! What is real and what is not? Who made up what? And if 'Sasuke' exists in the real world, in the shape of Dr. Edwards (if that world is in fact the real world), does Sakura and the others too? So many questions I want answered! Yes, this is definitely addictive. Keep it up! ^.^

Wow, this is really good! Never saw anything like this, but it does make so much sense with it all existing inside someone's head... In a way it reminds me of a book called 'I never promised you a rose garden', where a girl/young woman enters her private world every now and then. It has native americans in it, and it all gets complicated because she does to herself what she believes they do to her inside her head... I hope you continue this one. ^-^
